Community Health Care Workers (CHW) serve the community as a bridge between the member (community) and the healthcare system (providers) through outreach and education. By providing these services, CHW's help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other. This position is part of a new initiative in addressing members needs who are most vulnerable.
This role requires face-to-face visits within Cook County, Illinois
As a Community Health Care Worker at Aetna you will:
Work on an extended larger team that includes a Community Health Educator (CHE), Nurse Care Managers, and other CHW's. As a team you will work to provide needed resources, fill gaps, and remove barriers so that the member will be successful in managing their health care needs.
Meet with members in their home to conduct a brief Needs Assessment
Participate at local vaccination community events (Covid, Flu, and/or Shingles)
Participate in health fairs that provide BP, A1C, and cholesterol screenings.
Collaboration with CBOs and Providers (FQHC, Department of Health, and traditional providers)
Service coordination and navigation
Assist members with program application completion that addresses Social Determinant of Health issues and barriers
Educate members on Life Skills such as budgeting, food shopping, and/or food preparation
Educate members on Healthy Living, Physical and mental health well being, Tobacco free drugs, and/or alcohol.
Assist the member with referrals for services in the community
Document work with member in a database system and review as a team
Required Qualifications
2 plus years' experience providing clinical services; experience in community/outpatient setting as well as Public Health.
Bilingual (Korean Mandarin, Cantonese, or Vietnamese)
Must be willing to travel within the Cook County, Illinois area up to 50% of the time.
Must reside in Cook County, Illinois
Preferred Qualifications
Experience in serving the Medicare Populations in urban or rural environments with familiarity of resources within Miami-Dade County.
Health Care Condition knowledge of Diabetes, Hypertension, and/or Cardiovascular Disease
Experience using a computer including the use of Microsoft Office and Electronic Health Records
Bachelors in Community Health and/ or Social Work

Pay Range
The typical pay range for this role is:
$18.50 - $38.82
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
