Community Health Care Workers (CHW) serve the community as a bridge between the member (community) and the healthcare system (providers) through outreach and education. By providing these services, CHW' s help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other. This position is part of a new initiative in addressing members needs who are most vulnerable.

Required Qualifications

• Must reside in Trenton or Edison , New Jersey

• 2 years' experience providing community health care services; in community/outpatient setting preferred. Community Health Worker

• Experience with Microsoft Office

Preferred Qualifications

• Experience in serving the Medicare Populations in urban or rural environments with familiarity of resources within Central New Jersey and vicinity

Education

High School Diploma or equivalent G.E.D.

Pay Range

The typical pay range for this role is:

$18.50 - $42.35

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
