    Hours for this position are Monday-Friday 9:00am - 5:00pm.

    The Discharge Coordinator is an integral member of the interdisciplinary population health team and accountable for the support of ACO members who are discharging from the hospital and do not have a follow up appointment. The duties of this individual will be as follows:

    • Engagement with hospital and primary care team to offer support for coordinating discharge follow-up for primary care via Epic Chat

    • Secure PCP appointment within 2 weeks from the date of admission; cancel and reschedule if hospitalization ends up being prolonged

    • If patient does not have an established primary care provider, assist patient to make such a connection and access care

    • For any patients who are taking more than 10 medications, additionally schedule a pharmacist transitions of care visit in addition to PCP visit

    Patients within the Discharge Coordinator's care will have at least one priority condition but not a penalty diagnosis and is likely to have some recent history of hospital admissions in the past six months. Critical to the success of this individual will be the desire to support the optimization of care for vulnerable adults. This position will include a hybrid of on-site work, community-based home visits and accompaniments and remote work.

    Required Qualifications

    • 3+ years experience in health-related field.

    • 1 year of experience within Healthcare and/or community-based experience working with diverse populations with complex chronic illness and/or behavioral health population

    • Bilingual (English/Spanish)

    • 1 year of experience hospital documentation and scheduling systems and care coordination applications

    • Additional employee health clearance required by client

    Preferred Qualifications
    • Computer proficient with outlook, windows, and word

    • Excellent oral and written communication skills and organizational skills
    • Experience collaborating with medical professionals

    • Ability to foster a supportive environment for beneficiaries and work collaboratively with a multi-disciplinary team

    Pay Range

    The typical pay range for this role is:

    $19.52 - $40.90

    This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
