    • The Nursing Facility to Community Transition Money Follows the Person (MFP) Liaison is a supervisory role responsible for the daily oversight and operations of the Nursing Facility to Community Transition and MFP program. The Nursing Facility to Community Transition MFP Liaison shall be responsible for the management and supervision of the MLTSS Transition of Care (TOC) Team, the MLTSS Housing Specialist (HS) and Member Advocate. The Liaison will be responsible for the adherence of the State required Nursing Facility Transition Protocol, and ensure the market remains eligible for the Nursing Facility Transition State incentive. The Liaison will oversee staff who are involved in actively transitioning members from the Nursing Facility/Assisted Living to the community. The Liaison will be dedicated to the Managed Long-Term Services and Supports program (MLTSS), including members enrolled our MLTSS FIDE SNP Program.

    • Oversight of the TOC staff will include timely identification of members who wish to transition to the community, coordination of onsite visits to support Interdisciplinary care Team meetings, and coordinate transitions needs with the NF/SCNF staff, the MLTSS CM, MLTSS Housing Specialist, Member Advocate, State partners, community providers, and coordination of service delivery.

    • Successful completion of the NJ Choice certification is a contingency of employment. As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.

    Required Qualifications

    • Must have an active unrestricted NJ State RN license or an active Licensed Social Worker Certification in NJ.

    • Minimum of 5 years of experience serving the Medicaid/Dual population; with a focus in the Elderly and Disabled population.

    • Minimum of 5 year of previous leadership experience and oversight of operations.

    • Must possess reliable transportation and be willing and able to travel up to 10% of the time. Mileage is reimbursed per our company expense reimbursement policy

    • Successful completion of the NJ Choice certification is a contingency of employment. As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.

    • Excellent assessment, writing and communication skills are required.

    • Proficient in excel and analysis of trends.

    • Discharge planning

    Education

    • Master's Degree for Licensed Social Worker
    • BSN for Registered Nurse

    The typical pay range for this role is:$96,408.00 - $207,648.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
