Position summary

Position Summary: Responsible for assessing the risk and capacity of individuals referred by the community and addressing the needs of referred individuals.

Principal Responsibilities:

  1. Conduct assessments to determine client risk and capacity.
  2. Establish an appropriate, comprehensive care plan for the client.
  3. Provide information designed to meet the needs of the client.
  4. Make appropriate and timely referrals and arrangements for services.
  5. Be responsible for the program’s follow up on all care plan items.
  6. Ensure that all New York State Office of Children and Family Services and New York City Human Resources Agency contractual obligations and agency policies are met.
  7. Advocate on behalf of the client to ensure that they receive everything to which they are entitled.
  8. Consult with and keep supervisor informed of needs and problems related to the client.
  9. Provide crisis intervention to stabilize client situations.
  10. Deliver cash, checks, food, mail and other items to the client as needed.
  11. Maintain meaningful contact with clients at least once a month.
  12. Visit clients in their homes on a monthly basis to assess their living conditions. 
  13. Participate in internal and external meetings to ensure that client needs are addressed and appropriately coordinated.
  14. Ensure that all involved individuals and agencies are updated timely regarding client status.
  15. Assess for, initiate and follow through on discharge planning as needed.
  16. Request, follow up on and facilitate heavy duty cleanings.
  17. Attend and participate in court hearings.
  18. Prepare and maintain required documentation.
  19. Complete reports, applications and statistics thoroughly and accurately. 
  20. Meet deliverables by the due date.
  21. Maintain a consistently professional demeanor when communicating with clients, peers, supervisors and collaterals.
  22. Document all interactions in case notes.
  23. Ensure that client file is updated, accurate and complete.
  24. Participate in individual and staff supervision, as well as training, as assigned by program management.
  25. Provide on-call services during out of office hours, on rotation with other staff.

Salary range : 43,000 - 46,000 per year commensurate with experience

Job Competencies & Minimum Qualifications:

  1. Bachelors required in a related field
  2. One-year related experience and/or training
  3. Working knowledge of Microsoft Office Suite and other technology
  4. Excellent communication and listening skills
  5. Excellent customer service skills with a focus on treating clients with respect and dignity
  6. Able to multi-task and work independently with a great attention to detail

Working Conditions/Physical Demand: Business office environment with phone and computer use; travel via public transportation. This position may have the ability to work from home 2-3 times per week.
