What you'll do...
Evaluates ongoing patient health status and quality of life at intervals based on medical education, training, and experience by conducting patient evaluations; calling patients to ensure adherence to medications and resolve medication issues; collecting drug and disease information; communicating and reviewing new medication instructions with the patient; advising patients of side effects, dosage, storage, and administration of medications; and advising patients on how to treat possible side effects.
Collects information for specific diseases by administering validated questionnaires to evaluate ongoing patient health status and gauge treatment response; determining how often to administer questionnaire to patients based on medication cycles and guidelines; and distributing information to treatment staff, pharmacies, and prescribers.
Ensures new patients receive medication information and supplies by organizing and distributing information packets for new patient prescription fills; verifying new patient prescription packets needed for shipments; ensuring patient starter kits are included in initial shipments for specific drugs; and logging starter kit distribution for manufacturer and facility.
