Executes both routine and non-routine business support tasks for the Community Health area under limited supervision, referring deviations from standard practices to managers.
Follows area protocols, standards, and policies to provide effective and timely support.
Builds and maintains effective working relationships with members, providers, case managers, and agency representatives, and assists with care plan implementation.
Manages initiatives to reduce culture and socio-economic barriers between members and providers and identifies gaps and opportunities to strengthen systems of care with the community.
Maintains member health records, compiles reports, develops educational materials, and completes program documentation.
Takes direction to execute techniques, processes, and responsibilities.

Working knowledge of problem solving and decision making skills

Not certified but will obtain CHW certification within one year of employment

The typical pay range for this role is:$18.50 - $31.72This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
