Why don't you practice mock interviews, and rehearsals with a peer, manager, or reference so that you can monitor and better control your anxiety when interviewing?

Have you tried a life coach? I have had one for a few years now, and she and I have tackled some pretty thorny issues together with great success. I swear that I never thought I was going to have a life coach because they weren't for 'people like me.' Turns out, I was completely wrong. Otherwise, perhaps you could practice role playing interviews on the phone with some trusted friends. If you have a therapist s/he may be able to offer tools and guidance to help. Good luck--I am cheering you on.

I'm not totally sure what you're asking here. Am I correct in assuming that English may not be your first language? If communication is an important part of your job, which it is if you are a project manager, you'll want to polish up your communication skills. If you're providing info in writing, make sure you proofread everything and if possible, maybe run it by someone else to make sure it clearly says what you want it to. If it's being anxious or nervous on a video call that is the problem, do everything you can to relax before and during the call... Stretch, breathe, have a cup of tea. Then during the call, take your time. Make sure to speak slowly and clearly because an anxious, rushed voice is harder to understand. Make sure that you've rehearsed a number of different possible interview questions and how you would answer them. Have some brief examples of your accomplishments ready to share if different questions arise. I hope this helps!
