I have been wondering what happens with this information. How does it get used by the hiring company? Does the hiring manager see it?
What is the implication of answering "prefer not to answer"?

In an HR training with an employment lawyer, I did learn that it is the responsibility of the employee to declare and request any reasonable accommodations for their disability. Reasonable has a very wide definition. This was in Minnesota.
I think it's up to you whether you think your disability is something an employer should know about so they can accommodate you. I agree with Joan that this information can't be used against you and they might not even have access before they interview you.
I've been using ATS systems since they were invented and I've never even thought to try to look at any of that information. Not even sure if I would have access to it. It's usually just collected and then reported to the government. I doubt hiring managers care or have access to it either.
