I am burnt out
I've been in my current role (at a company and on a team I love) for the past year, and it has been a lot, to say the least. I am one person doing the job of 2 teams, and have had little support or active leadership throughout the year. I'm in the process of transitioning to a new role, but in the meantime, I'm now supposed to take on new responsibilities and learn the new gig, while also still doing the old stuff with even less support.
I would love advice on how to handle this, or even just words of encouragement would be nice.
Yes, congrats on the new role and wow, what a lot on your plate. My advice for these times of constrained energy and time, is to try to get crystal clear on the result you want to create. For the hand-off you're preparing, what is it that you could do that would make you consider it successful? A readme file? A video training of the tasks you do? A few introduction calls and meetings? Get as explicit as possible. And then, let everything else in that/those roles go. If you have a clear result you're working towards, it makes decisions easier and loosens the grip of guilt for what doesn't get done. You'll be able to leave proud and with a clean conscience.

First, congratulations on your new role! I was in the same situation as you last year and started a new role at the beginning of this year. It was tough balancing multiple jobs with the new one, but there is light at the end of the tunnel, just keep thinking that :)
