Medical Insurance: You have 4 different medical options to choose from: Anthem BCBS PPO, $1,500 CDHP with HSA or $2,000 CDHP with HSA. The medical plans differ in level of coverage and employee cost. Once you enroll in a medical plan you cannot alter your selection, except during the annual Open Enrollment period. Payroll deduction may be elected on a pre-tax or after-tax basis.
Health Savings Accounts (HSA): If you elect to participate in the $1,500 Deductible Plan with HSA* or the $2,000 Deductible Plan with HSA**, you will be eligible for the Health Savings Account and company contribution.
Dental Insurance: We offer two Dental Plans. The plans administrator is Delta Dental of GA. Preventative dental is covered at 100% with no deductible. The annual deductible is $50 per person $150 per family for basic and major dental services. Basic services are covered at 80% and major services are covered at 50%. The annual plan reimbursement maximum is $1,500 (standard plan) and $2,000 (enhanced plan). Enhanced plan offers orthodontia benefits, which is covered at 50% with a lifetime maximum of $2,000 per person.
Vision: We offer two vision plans that provide coverage for eye exams, glasses and contact lenses. There is no co-pay for eye exams or for contacts. There is a $20 co-pay for prescription glasses. There is a $130 (standard plan)/$175 (enhanced plan) allowance for a wide selection of frames, or contact lenses when purchased in lieu of glasses. Standard Vision plan insurance is provided in full by the company at no cost to you.
401(k): You become eligible to participate in the Plan as of your date of hire as long as you are a full-time or part-time employee of the Company. Unless you elect otherwise, you will automatically be enrolled in the 401(k) portion of the Plan approximately 4 weeks from date of hire. This is called “auto- enrollment.” You will be put into the Plan at 3% before-tax. After your initial enrollment, you may change your contribution amounts or fund allocations at any time. When you save through the Plan, you have the extra advantage of receiving matching Employer contributions. Upon completion of one Year of Service, measured from the date that you first complete an Hour of Service, for every $1 you save―up to 8% of your compensation, your Employer will contribute an extra $ .75 to your account.
Mercedes Retirement Contribution (MRC): This plan is for employees hired after January 1, 2015 and who are active as of December 31. The company will make an annual contribution to the MRC based on the employee’s age as of December 31 of the plan year. The amount of the contribution that you are eligible to receive is based upon your attained age on the last day of the Plan Year and your compensation under the Plan that is taken into account. Your Compensation for this purpose is limited to normal, regularly recurring salary or wages which is paid to you while you are eligible to participate in the Plan. You become a participant in the MRC after completing one year of service. Please see chart for company contribution percentages.
Employee Assistance Program (EAP): MBRDNA offers a work life assistance program as a voluntary, free and confidential benefit for employees and their family members. Website: www.magellanhealth.com.
Tuition Assistant Program: Employees are eligible for assistance up to $6,000 per calendar year when enrolled in an undergraduate or certificate program. Enrollment in a graduate program qualifies employees for reimbursement of up to $8,000 per calendar year. The company additionally provides financial assistance to eligible employees who complete job-related courses at accredited colleges, universities and approved vocational or technical institutions.
Employee Referral: The MBRDNA Employee Referral Program is an opportunity for current employees to recommend our company to their network in pursuit of finding top talent to add to our team. Candidates must be referred to a specific position. Referrals must be submitted through the People Team, not directly to the hiring manager. The applicant must disclose the referring employee’s name on their employment application and complete 90 days of service with the company.
Healthy Roads Program: We all know that taking good care of ourselves is important. To reinforce our commitment to helping you achieve your health improvement goals with Health Roads wellness program, MBRDNA is offering you the opportunity to earn money for participating in a variety of activities.
Health Care FSA: This benefit allows you to have pre-tax dollars deducted from your paycheck and placed into an account so that you can be reimbursed for any eligible health care expenses, incurred by yourself and eligible dependents, which are not reimbursed by the medical and/or dental plans. The minimum annual deposit is $240 and the maximum annual deposit is $2,750. If you enroll in the CDHP medical plan you are not permitted to enroll in this FSA account.
Wellness Reimbursement: MBRDNA encourages employees to participate in a variety of activities aimed at improving overall wellness. Full-time employees and global assignees are eligible for reimbursement of fitness related activities and equipment up to a maximum of $500 per calendar year.

Supplemental Life Insurance: You may elect additional life insurance on yourself in the amounts of 1x, 1.5x, 2x, 2.5x, 3x, 4x, 5x or 6x your base salary, up to a maximum of $1,500,000. Evidence of insurability will be required for an election above 3x salary or an amount above $500,000. During the evidence of insurability process you will be covered for up to 3x salary or $500,000 until your election is approved by the insurance company.
Spouse Life Insurance: You may choose to purchase a life insurance policy on your spouse in the amounts of $10,000, $20,000, $30,000, $40,000 or $50,000.
Child Life Insurance: You may choose to purchase a life insurance policy on your children in the amount of $2,500, $5,000, $7,500 or $10,000.
MBRDNA believes that taking time off for vacation and other personal needs is essential to the health and productivity of every employee. We trust you to use your best judgment and be conscientious when deciding how to exercise your Flexible Time Off (FTO).
Paid Parental Leave (PPL): With one year of service at MBRDNA, employees are eligible for up to six weeks of 100% paid bonding leave (if less than one year of service, an employee gets three weeks). PPL can be used for employees who need time to bond with a new child entering their life by either birth, adoption, or foster care placement. This leave can be taken all at once or in weekly intervals.
Long Term Disability: The Long Term Disability Plan (LTD) is designed to provide you with 60% of your base monthly salary, (less any other disability benefits) should an illness or injury disable you for longer than 6 months. It is important to know that if you are enrolled in the medical and dental plans, coverage remains in force during your long term disability period. Ramp Back Program: MBRDNA offers an 8-week “Ramp Back” part-time work (30 hours/week) adjusted schedule to assist an employee’s transition back to work. Ramp Back time is used in a continuous one-time 8-week block for returning new parents fresh from their baby bonding leave.
Dependent Care FSA: This allows you to have pre-tax dollars deducted from your paycheck and placed into an account so that you can be reimbursed for any eligible dependent care expenses you incur because you, or if you are married, you and your spouse, are at work. For example, the cost of a day care center. The minimum annual deposit is $500 and the maximum annual deposit is $5,000 per family.
