It's going to depend on how an organization manages it and the maturity level of the individuals (at all levels) at the organization. I've seen studies where people with unlimited PTO use less because they don't want to be seen as a slacker taking time off. I've seen stories of managers showing preferential treatment when approving time off. I've heard of people working for managers who are "team no time off" and so the employees think that in order to please their manager they need to show that same drive.

Do you mean Unlimited PTO? The cons are most people don't use it. The company doesn't track it and they don't have to pay you for it when you resign.
