First off, my thoughts are with you concerning your diagnosis. I pray this is just a detour and you are back to your normal day to day in no time.
In saying that, I feel that how urgent is this position for the other company? Do they need someone to immediately step in and hit the ground running. If they do, is it going to be too much stress on you dealing with doctors appointments AND trying to learn a new position. Even if it is a similar job, a new company will always have different ways of doing things.
If it is determined that you will need extended time off, until you have been at your job a year, you will not be eligible for FMLA. Just keep that in mind before.
If this is your old boss and old coworker, I feel like you have a history with them and could be honest about it. I would just hate for you to get in there an them expect more out of you than you can give.
Regardless of the treatment, the mental toll that this can have on you is tough. Adding a new position will could make it worse.

As you say "they are likely very interested in hiring me" tells me that the process of speaking to them, interviewing, etc has not started. With that being said, do you honestly think any decision would be made in the next 2 weeks? You could still be in the process while you take care of your health - and you would start after you are able. I would at least start talking with them and see where it goes. I wish you luck
