Call them, don't burn the bridge and be as respectful of them as they were of you. Clearly if they were willing to hold the job open they care enough about you to be kind, give that in return.

Why give a heads up. Just give your notice and go. You have the right to be happy in your work. Trust me if they needed to cut staff they wouldn’t be hesitant to do so. You don’t owe them anything.
