Off the wall can be nonsensical or also really irate with dementia. Often if that's the issue, it's easier to not ask the person detailed questions about doing A vs B. Assuming you know what standard procedure is for a situation, saying "$thing is due,( or $thing happened); should we do x?" --try to structure questions where the person can give a yes or no or shorter answer.

I too am experiencing something very similar and I’m also at a loss over how to approach discussing the situation. Worse yet, I’m in a profession where malpractice liability is a concern and my concerns are increasing exponentially on a daily basis.
