If your company has an EAP program, contact them (if you haven't already) and make sure you get into therapy. You may have already done this, so I apologize for stating the obvious. As one who also deals with a tremendous amount of anxiety (associated with PTSD), I know how debilitating it can be. I also agree with the person below who suggested it may be altering your perceptions. Your description of how things spiral out of control is suggestive of this. Please take the time to heal and take care of yourself. If you need an extended medical leave, your student loans can be put into deferment. You can emerge from this and restart your career. It's much easier with a healthy state of mind, though.

Ok, so first off, great job on seeking the help you need and getting counseling! That is a great support system to setup and continue.
As for feeling like you aren't wanted at your current job, I'm reading a lot of assumptions and interpretations which are based on anxious thoughts. It seems like no one has actually given you explicitly bad feedback. The "boredom" thing could be your manager trying to check in and figure out how to engage you. I would discuss these feelings of exclusion and "wanting you gone" with your counselor. If you feel comfortable enough, it's something to potentially address with your manager to seek out accommodations as well.
Finally, for getting a new job. Your current company really doesn't have a say in the hiring process for the next job unless you list them as a reference. Future jobs will interview you and want to know about your professional experience and potentially why you're looking for a new job. Prep for interviews as usual, and apply a ton (even to places you think you only have 60% of the criteria). Your career goals and enthusiasm for that will shine through! I would also recommend prepping for interviews with your counselor and get strategies for decreasing anxiety of interviewing which is hard for everyone, but especially those with anxiety. Perhaps even medication is something to consider.
