Contact the sender. It could be a legitimate mix-up. If not, I hope that you have a police report from years ago, when you may have lost your credentials.
My niece has an unusual spelling of her name. She lost her purse. Someone in the workforce has assumed her identity. There is a police report filed. No one has caught up with the offender.

Hello Anonymous, I appreciate and thank you for your concerns. I have never lost my credentials. Last week, I received a call from someone pretending to be from the Social Security Administration / US Marshall. With the widespread Social Security fraud scams across the country, I hope and pray
This matter is easy to get straight out with the IRS.

Yolanda - Are you sure it isn't a from a company you may have done work for that is based out a different state? Maybe the 1099 was sent by their corporate office.
There is a lot of unemployment fraud going on right now though I haven't heard anything about 1099s. It's interesting they had your correct address too.

Hi Alexis,
No, it’s from a state I have never done business within Ohio. Yes, you are correct with the Pandemic; there is a lot of unemployment fraud going on.
I have reported it to the Ohio Department of Job and Family Services, which is what the letter instructed me to do in case of fraud. Yes, they had my correct address, which is why I’m of such concern. It looks like someone has a lot of time on their hand with identity theft.
