Whoa, whoa, whoa. This is alot to bear. I am sorry this has been your experience. I literally can feel the pain and understand it fully.
Unfortunately, whether it is due to jealousy, animosity, self-hatred, or even domestic violence at home, some bosses stoop to this level of toxicity everyday, without any justifiable reason or cause.
It appears you have already done the introspection and know everyday you give your job 110% and your boss is averse to everything that you do.
For now: Continue to be humble and keep the peace. Your boss does not come across as someone you can reason with and have a healthy discussion with to build a healthier work relationship that involves reciprocated respect and dignity.
Exist Strategy: Polish that resume and cover letter. If you can move laterally in the same environment to create distance between you and your boss do so. If there are similar roles outside your org also apply to them. Since she is your boss, find other execs, leaders who can vouch for you and include them as references on your resume to help offset anything your boss might try to say to block your ability to move on. List those who can vouch for you as "primary contacts" and list them first before your boss on your resume.
