I have had multiple occasions where I meet an employer for a pre-interview, get asked for a follow-up interview, and then soon after, receive a rejection message. Does anyone know why this happens? It's been a bit frustrating and disheartening for me because I know I am qualified.

It happens but what you can do is send an email thanking them for their time and if they wouldn’t mind, ask for some feedback on your interviewing skills and if they could share any feedback - why not?

Why this happens? Sometimes they have an internal candidate. The ATS software often automatically pushes out rejection notices when a candidate is selected for hire.
