I work in healthcare as a nursing leader, specifically in the hospice field. If any of you have ever worked in hospice, for a for-profit agency, you know it is incredibly taxing! I have been here for almost two years. There is always a huge push to continue admitting patients even when our staffing level is extremely stressed. Staff leaving due to the heaviness and non-existence work/life balance with still a big push to admit, admit, admit! I almost left about a month ago, I gave my notice, cleaned out my office, and everything! Then my boss came to me with an offer for a promotion and a $20K salary increase. I agreed to stay. So far, nothing but my title has changed. I didn't really get a promotion, they just gave me a higher level title with a big salary bump. I am being considered to move into a director role back in the acute care world. Does it make me an absolute awful person if I accept this new position and now give my notice again, but for good this time?! I feel really guilty! Healthcare is tough all around but this for-profit stuff is really killing me!

Do what is right for YOU, guilt-free. It's not selfish to take care of ourselves in situations like this

Unfortunately this is healthcare in general, not just home care/hospice agencies. Organizations are always looking to expand service lines to increase revenue, despite provider shortage (physician, extenders, nurses, unlicensed assistants, etc).

You are not an aweful person, if you opt to move on from a position that no longer meets your professional goals or causes you great distress

What do you feel guilty about?

I am also working in healthcare and everything you describe sounds so familiar to me.

You are so deep in this that a step towards a less draining job you are considering as an awful behaviour. It's great that you're empathetic and caring about so much about others. And it's great that you are working in nonprofit. But there is one person you may also need to take care of: it's yourself.

I am saying this because by being in similar situation and have the dilemma between staying and helping and enduring OR taking self-care and leaving, I experienced a burnout. So it's clear what to do.

I have read that nonprofit org. are very toxic. So the salary raise was a move to stop you from leaving but it's not surprising that nothing else changed. They could have given you that salary in the first place. I guess that it's an organisation with a high turnover rate. And that's why you feel even more guilty that you're considering leaving. It might feel like a betrayal but it's not. You're betraying yourself if you stay.

    I am talking from experience.

It is amazing that you state that you are having staffing issues. I actually went back to school because my family utilized hospice for a loved one and it was an amazing experience.

I find it nearly impossible to get into the industry. I am not a CNA, LPN or RN. But I do have a degree in Gerontology (study of aging). I had a professor at my school stated he received the same roadblocks.

There are people that want to work and help but most of us are not sure how to do that with all the games just to obtain a job now.
