I think most companies only provide start and end dates. Do companies include reason someone is no longer working if called for a reference check?

Are you asking about an employment verification or a reference? Employment verification only consists of start/end dates and the title upon termination of employment. In some states, they can ask if you are eligible for rehire.

If you put a person from a company as a personal or professional reference, they can ask about anything they want. This requires you to sign a release allowing them to contact them for a reference. This release requires you to provide their name, title, contact information (email/phone), and whether it is personal or professional.

I have included it. They always ask so I save the trouble. Plus they see I'm transparent

    And you could get sued.
