How do I handle a written warning from my company of 20 years that is false?

We are a publishing company and one of my reports didn't look at something closely enough (he was also undergoing cancer treatment) and books got published with 200 errors in them. The facts in the write-up are untrue and said I used poor judgement by giving him the assignment. I don't give him assignments on this project and I'm not even on the project. I suppose I will sign it, but want to include a response in case it gets legal. That was not welcomed by HR and she defended what was said (kangaroo court). Advice is appreciated! M
